Overview

The internationally recognized UCSF Advanced Heart Failure Evaluation and Therapies Program now offers a convenient outreach clinic in Fremont. Beginning in November 2014, the clinic, located at the Washington Township Medical Foundation offices, is designed to provide a convenient consultative service for our cardiology colleagues by expanding the spectrum of therapeutic options for advanced heart failure patients in the local community. A major goal is to identify patients early who may benefit from advanced heart failure therapies such as heart transplantation or mechanical circulatory support (MCS) devices. This will allow a convenient collaboration between local cardiologists and their colleagues at UCSF Medical Center.

Heart Transplant Program

With more than 20 years of experience in heart transplantation, the UCSF Heart Transplant Program is one of the most experienced programs in the region with excellent outcomes. Although there are established criteria for heart transplant candidacy, UCSF, as an academic medical center, has greater access to progressive devices, making more patients eligible for transplant.

Modalities incorporated in treatment of patients with Advanced Heart failure include:

• Imaging modalities, such as 3-D echocardiography, cardiac magnetic resonance imaging, cardiac positron emission tomography and coronary computed tomography.
• Invasive/interventional procedures, including right- and left-heart catheterization, myocardial biopsy, coronary angioplasty, valvuloplasty and alcohol septal ablation.
• Cardiac surgeries, including mitral valve repair, ventricular reconstruction and high-risk coronary artery bypass.

Volumes and Outcomes

• The UCSF Advanced Heart Failure Evaluation and Therapies Program has some of the best outcomes in the nation.
• Each year, 18-25 heart transplants are performed at UCSF, with more than 260 patients transplanted to date (including 12 heart-lung and 24 heart-kidney transplants).
• Recent national data compiled by the Scientific Registry of Transplant Recipients reported that the one-year survival rate for heart transplantation at UCSF was 94 percent compared to an average national rate of 91 percent and that the three-year survival rate was 97 percent compared to a national average rate of 83 percent.
• UCSF has one of the lowest readmission rates for heart failure patients in the nation, with an average all-cause, 30-day readmission rate of 11 percent. Our 90-day readmission rate, at 24 percent, is also one of the best in the nation.
• Inpatient and 30-day post-discharge mortality rates for acute decompensated heart failure patients are lower than the national average.
