Care Card


Medical information at your fingertips

If someone asked you tomorrow, would you know your blood type off hand? What about your allergies? Could you generate a list of every medication you can't take, or those that you do? Is your physician's phone number as familiar to you as your own? Of course it's not, but in case of an emergency, your doctor's number may be the one you need most. That's why we've created the Washington Hospital Care Card, a medical identification card that lists crucial emergency information and fits in your wallet just like a credit card. This special service is offered free-of-charge to anyone who is interested.

A patient may update the card's information at any time:  through a password-protected and secure website, at the point of service or via a mail-in form.  At all times the patient has absolute control over personal information in the database.  In order to protect the patients' right to privacy, both participation in the program and the information contained on the card are determined solely by the patient and will be maintained in a secure and confidential database.

Child Care Card

If you're a parent, you certainly understand the anxiety that comes with leaving your child with a babysitter. Washington Hospital's Child Care Card is designed especially for your little one. It gives your sitter important emergency info - and gives you peace of mind. It even includes space for the names and numbers of those you want contacted in an emergency, right at your fingertips.

Senior Care Card

On the flip side, you may worry about your aging parents or grandparents, especially if they have pre-existing medical conditions. At Washington Hospital, we care, too. A special Senior Care Card will alert healthcare providers of their specific needs, especially in case of an emergency.
